You are able to make a direct self-referral to Smart Paediatrics Occupational Therapy.
It takes approximately 15 to 20 minutes to complete this form thoroughly. Please add details to support your referral and ensure all information is correct.
After submitting your form, you will be invited to book in a free 15 minute Welcome Phone Call. You can then speak directly with the Therapist to ensure this is the right service for you.
The service has standardised assessments for children from the age of 5 to 12 years. Referrals outside of this range will be considered on an individual basis.
The service is based in Aylesbury, Buckinghamshire and welcomes referrals from all over the country.Â
